im not sure about pulling the first photo, but I was also confused with the plugin the no thumbnail appearing. I then saw a “featured image” when creating my post, towards the bottom right. I clicked it and took me to all my media. I then select a picture, update post and bam I saw the thumbnail in my post when inserting the “shortcode”. I hope this help.
